Introduction

Terminal Ileitis is an inflammatory condition of the terminal portion of the ileum. It may occur acutely with right lower quadrant pain, bleeding followed or not by diarrhea. It is also known as crohn’s disease.

Ayurveda Concept

In Ayurveda, we can correlate this disease with Grahani. Toxins (ama) as a result of vitiated doshas, mainly pitta and Vata – increase with consumption of sour, bitter, dry and spicy foods. Alcohol, tobacco and caffeinated beverages are also some of the contributed factors. Anxiety, stress, and anger also additionally worsen heat within the body and lead to inflammation.

Ayurveda Master, Sushruta says – 6th pittadhara Kala lies in between amashya and pakwashya which is called as Grahni and this makes a major part of GIT (Gastro-Intestinal Tract)

Epidemiology

  • The prevalence number of people having terminal ileitis is increasing steadily during the last 50 years in the United States.
  • This disease is slightly more common in males than females.
  • 7, among every 1 lakh people are affected with this disease.

Etiology

The cause of terminal ileitis is not known, as we do not have any proven theories about the cause but it is believed that it is caused by the factors listed below, as per modern science

  • Genetics
  • Environmental Factors
  • Immune System Problems

1. Genetics

Brothers, children, sisters, parents affected with this disease are more likely to develop the disease themselves. The disease is more common in certain ethnic groups like people of Jewish descent and whites.

2. Environmental Factors

Environmental factors can also lead to the development of the disease. These factors may trigger the immune system and may damage the intestines directly to have terminal ileitis. These factors may include

  • Cigarette smoke
  • Microbes like viruses or bacteria
  • Substances from something that has been consumed.
  • Key factors which are still unknown

3. Immune System Problems

The immune system protects our body from harmful microbes such as viruses, bacteria, fungi etc. and the body does not react to all microbes and hence, the immune system leaves them untouched. But the immune system of people suffering from this disease reacts inappropriately and the inflammatory response in the affected people does not stop and inflammation continues which results in ulcers to the intestine.

Terminal ileitis is intermittent/ sporadic. It means that it is impossible to predict or we can say it is highly unpredictable that when this condition will flare up, how long the flare will last and when it will flare up again. Diagnosis

Terminal Ileitis can be diagnosed with the help of history taking, physical examination, imaging studies, lab tests. As symptoms appear gradually and affect different parts of the intestine at different periods of time, it becomes difficult to diagnose. The various tests to diagnose terminal ileitis are as follows

  • Abdominal x-ray for locating the abdominal obstruction.
  • Flexible sigmoidoscopy / colonoscopy. The colonoscopy is the most preferred one among the two because it examines the entire colon
  • Upper gastrointestinal endoscopy
  • Upper gastrointestinal series
  • Barium enema x-ray
  • Computed tomography scan
  • Standard blood tests and urine tests
  • Video capsule endoscopy

5. Coolstrin B

Diet and life style in Terminal Ileitis

  • Diet plays an important role for every disease in Ayurveda with consideration that herbs work very well in alkaline body.
  • Drink coconut water, pomegranate juice, pumpkin soup, carrot juice and soup.
  • Take banana, apple, and pear. Avoid all citrus fruits like orange, lemon, grapes and sour berries.
  • Prefer all gourd vegetables like round gourd, ridge gourd, and bottle gourd. Pumpkin, bitter melon, carrot, potatoes, turnips are also good to have.
  • Avoid all dairy products like milk, cheese, buttermilk. Take only cow ghee and fresh curd.
  • One can take white rice and wheat roti, even corn flour roti is good to have.
  • Green and orange lentils which digest easily need to be preferred. Avoid or at least limit the consumption of kidney beans, chick peas and black grams.
  • Avoid packed, junk and fried foods.
  • Avoid alcohol and smoke.
  • Practice simple breathing exercises, anulom-vilom etc.
  • Read good story books or spiritual books to divert mind and combat stress.
