What is Aplastic Anemia?

In Ayurveda, Aplastic anemia is correlated with Pandu roga that develops due to the depletion of rasa dhatu (tissue element) which reduces the production of rakta dhatu. In Ayurveda, it is described in three forms i.e., in the form of a disease, complication and a sign of certain diseases. The common symptoms related to aplastic anemia are daurbalya (weakness), shrama (fatigue), adhirata (irritability), hridyaspandana (heart palpitation), vaivarnata (pallor skin), aruchi (anorexia) and shotha (edema).

Acquired aplastic anemia is a hematologic disease associated with mortality and morbidity. Destruction of hemopoietic stem cells plays an important role in its pathogenesis. This comprises a group of pathogenetically heterogeneous bone marrow failure. Bone marrow failure is caused due to the exposure of ionizing radiation. Anemia is normocytic, mild macrocytosis and can also be observed in association with elevated fetal hemoglobin levels and erythropoiesis.

According to this shloka, the prodromal symptoms of pandu roga are:

  1. Increased heartbeat
  2. Dryness in body parts
  3. No perspiration
  4. Fatigue

What are the Causes of Aplastic Anemia?

Aplastic anemia occurs when there is damage occurs to the bone marrow that slows down or shuttles down the production of new blood cells. Bone marrow is the spongy and red material present in the bones that produce stem cells which gives rise to other cells. Stem cells in it produce blood cells such as white cells, red cells, and platelets. In this condition, the bone marrow is described as hypoplastic or aplastic which means that it contains very few cells or it is empty.

Factors that cause the damage to the bone marrow which may  be temporary or permanent and affect the production of blood cells are :-

1. Exposure to Toxic Chemicals

Pesticides and insecticides may also cause Aplastic anemia. This type of anemia may get better on its own if the patient avoids the repeated exposure to the chemicals.

2. Autoimmune Disorders

These are disorders in which the body’s immune system attacks the healthy cells that may involve the stem cells in the bone marrow.

3. Radiation and Chemotherapy Treatment

These treatments kill the cancer cells but may also damage the healthy cells which include the stem cells in the bone marrow.

4. Viral Infection

Viral infections play an important role in the development of Aplastic anemia. Viruses that develop aplastic anemia include HIV, hepatitis, cytomegalovirus, parvovirus B19, and Epstein-Barr.

5. Pregnancy

Weak immune system during pregnancy may attack the bone marrow.

Unknown Factors

There may be an unknown cause in many cases. This is termed as idiopathic aplastic anemia.

What are the Symptoms of Aplastic Anemia?

This condition may progress slowly and may take over weeks or months or may come suddenly. The condition of the disease may be acute or chronic. It can be very severe and even fatal.

Symptoms of aplastic anemia may include:

  1. Pale skin
  2. Skin Rashes
  3. Headache
  4. Dizziness
  5. Shortness of breath with exertion
  6. Irregular or rapid heart rate
  7. Easily bruising
  8. Bleeding from nose and gums
  9. Fatigue and weakness
  10. Prolonged infections and bleeding from wounds and cuts

What are the Risk Factors of Aplastic Anemia?

Several factors that may increase the risk of aplastic anemia include:

  • Pregnancy (rare)
  • Certain autoimmune disorders and infections
  • Use of drugs such as chloramphenicol as it is used to treat bacterial infections and rheumatoid arthritis.
  • Direct exposure to chemicals
  • High dose of chemotherapy or radiation therapy.
